Stephens Co. Health Department encourages residents to complete health assessment

Stephens and Jefferson County Health Departments have partnered with DRH Health and Pathways County Health Improvement Organization to conduct a Community Health Needs Assessment. 

The information collected during the assessment period helps to inform what, where, and how services and/or resources should be provided within the community over the next 3 years.

All residents are encouraged to participate in the Community Health Needs Assessment survey by scanning the QR code or visiting surveymonkey.com/r/R6h3ZSM.

This anonymous survey allows local health departments and organizations to better understand health issues, challenges and needs the community faces. Surveys are collected every three years to identify and address health and social needs.

The survey is provided by the Stephens County Health Department, Jefferson County Health

Department, DRH Health, and Pathways County Health Improvement Organization.
